Caregiver Support Program

Our unique Caregiver Support Program is designed for those people who want to take care of their loved one during a health crisis, but who may find it overwhelming.
You have the desire and compassion to provide that much needed one-on-one care, but aren’t sure if you are doing things properly or effectively.
Malibu Health and Home Care will work with you during this challenging time by coaching and training you on providing the proper care. We will always be around for guidance and support and will provide you with the necessary tools to do a great job!

The Caregiver Support Program includes all of the following services for one low price:
• Care Management Services, including Initial Assessment and weekly visits with an experienced Client Care Manager;
• Training and review on basic personal care techniques;
• Training on Retire-At-Home documentation;
• 24-hour on-call support for questions and urgent matters;
• Communication with other health care professionals;
• Aid in navigating the health care system;
• Arrangements for Home Medical Equipment and Supplies;
• Immediate response for additional care.

Peace of Mind Package
Knowing your loved one is safe is the peace of mind you need!
Our unique Peace of Mind Package is designed for those people who are caught between the desire to give mom and dad the extra help they need, and the reality of trying to balance their own work and family life. Its tough juggling everything, and everyone could use a little help!
The Peace of Mind Package will help identify when your loved one’s needs are changing and be proactive on safety issues so that we can address things before they become an issue. Many of us deal with one crisis after another when it comes to an ill loved one and it is through constant reaction to situations when life’s priorities can become unmanageable.
The Peace of Mind Package includes all the basic requirements your loved one might need to help get through the week while you’re not around. Your Peace of Mind comes from knowing that an accredited, experienced home health care company can provide home support and personal care, on-going supervision, 24-monitoring and communication with you – all for one low price! We keep things simple so that Mom and Dad can be safe without feeling too overwhelmed!
The Peace of Mind Package includes all of the following services for one low price:

• 3 hours per week of Personal Care Services;
• Monthly review visit by experienced Client Care Manager;
• Weekly phone call from Client Care Manager;

Respite Care Package
Rest and Recharge. It will improve your ability to provide care!
We all deserve a break. Even though you take great joy in providing care to your loved one, the physical and emotional consequences can be overwhelming without taking a break once in a while. Research has shown that receiving some respite can have a positive effect on the health of the caregiver and has been shown to help sustain family caregiver health and wellbeing.
Malibu Health and Home Care has designed our Respite Care Package with you and your loved one in mind. Our services are tailored to the specific needs of your loved one, while ensuring that the management of the care meets or exceeds your expectations so you can rest comfortably.
We are an accredited organization that has a long-standing reputation of personalized, professional care. You can rest assured that your loved one is receiving top quality care while you take a short rest and recharge your batteries.

Hospital Discharge Package
Taking the stress out of going home!
We understand that coming home from the hospital is a much-anticipated moment, but not without its stresses for you and your family. Malibu Health and Home Care can help you with support and services that help make this transition home as safe and comfortable as possible.
There is a lot of preparation and coordination that must occur in order to ensure you travel safely home and that everything you need is waiting for you. There are meds to be arranged, follow-up appointments to be booked and at-home recovery instructions to be managed, not to mention transportation and other important issues.
By calling us directly, one of our experienced Client Managers can meet with you and your family while you’re still in the hospital to discuss a plan and coordinate everything with your hospital Social Worker. We will take time to understand your individual needs and design a plan that will take the stress out of moving home so that you can concentrate on a successful recovery!
Our Hospital Discharge Package includes all of the following services for one low price:
• Care Management Services including In-hospital assessment by an experienced Client Care Manager;
• Attendance at discharge planning meeting;
• Visit to your home prior to discharge to ensure your home is properly prepared;
• Coordination and delivery of Home Medical Equipment and Supplies;*
• Arrangement of your transportation home;
• 10 hours of Personal Care Services and/or Home Support Services;
• 24-hour on-call support for questions and urgent matters;
• Immediate response for additional care;
• Communication with other health care professionals.

Live-In Management Program –
You already have a private live-in caregiver, but would like a professional to manage the care they provide.
If you already have a caring, trusted, reliable, live-in caregiver that helps you with all your daily needs, we can enhance the care he/she provides by guiding them and coaching them on the more advanced aspects of caregiving.
With weekly visits to your home, our Client Care Managers review the care you receive ensuring complete satisfaction and peace of mind that you are truly the care you need and deserve!
Our Live-In Management Program includes all of the following services for one low price:
• Care Management Services including an Initial Assessment with an experienced Client Care Manager;
• Weekly visits by an experienced Client Care Manager;
• Training and review on basic personal care techniques;
• Training on Retire-At-Home documentation;
• 24-hour on-call support for questions and urgent matters;
• Communication with other health care professionals;
• Aid in navigating the health care system;
• Arrangements for Home Medical Equipment and Supplies;
• Immediate response for additional care.
